A Path Forward, Together

Walking Alongside Families

The Family Case Management Program assists individuals and families facing barriers to accessing community resources and navigating systems, particularly those with limited support networks.

Bilingual case managers work with each client to develop a personalized plan based on their needs and goals. Services include:

  • Case management and referrals to community partners
  • Referrals to legal orientation and immigration-related resources
  • Mental health and reduced-cost healthcare referrals
  • Employment and job-search support
  • Assistance with completing applications for available government benefits and community resources
  • School enrollment and navigation of community services
  • Emotional support and ongoing guidance

When available, the program also provides financial assistance for urgent expenses. Assistance is reviewed and approved on a case-by-case basis, depending on the client’s needs and available funding.

Our approach is individualized and centered on kindness, dignity, and respect, helping each client build stability and move toward self-sufficiency.
