2021 Run/Walk For The Six Challenge – MEDICAL WAIVER You understand that participating in the 2021 Run/Walk For The Six Challenge as a member of the JCS Alliance Team Blue Card, including pre-event training and related event activities (collectively referred to as the “Event”) is a potentially hazardous activity. You agree not to participate unless you are medically able and properly trained. You are voluntarily entering and assume all risks associated with participating in the Event, including, but not limited to, falls, spills, contact with vehicles, other participants, spectators or others in the area, effects of weather, including cold or extreme heat and/or humidity, traffic and the conditions of the roads, all risks being known and appreciated by you.
In the event of an illness, injury, or medical emergency arising during the training period or during the course of the Event, you agree to be fully responsible for payment of any and all medical services and treatment rendered to you, including but not limited to medical transport, medications, treatment, and hospitalization.
As it applies to your participation in this Event, you agree to abide by the Center for Disease Control (CDC)’s recommendations for the prevention of the spread of COVID-19 and attest to having read the CDC’s guidance at: https://www.cdc.gov/coronavirus/2019-ncov/prepare/prevention.html. You also agree to abide by any COVID-19 distancing and other safety guidelines issued by the state, the community or by this race for my participation in this race.
Having read this Waiver and knowing these facts, you, for yourself, and anyone entitled to act on your behalf, waive and release The Blue Card, Jewish Community Services of South Florida, and its constituent chapters, all Sponsors, of the Event and of the Team, and each of the respective representatives and successors, officers, directors, members, agents and employees of the foregoing, from all present and future claims and liabilities of any kind, known or unknown, arising out of your participation as a member of the Team Blue Card, even though that liability may arise out of ordinary negligence or fault on the part of persons named in this Waiver.
MEDIA RELEASE You grant permission to Jewish Community Services of South Florida and The Blue Card, its agents, contractors or representatives, to use, or authorize others to use, my name, voice and images of myself in any photographs, motion pictures, results, publications or any other print, videographic or electronic recording of this event for legitimate purposes as a member of the JCS Alliance Team Blue Card for any legitimate purposes without remuneration.
This event follows the standard running industry policy: All entry fees are non-refundable. We reserve the right to postpone or cancel the event due to circumstances beyond our control such as a natural disaster or emergency or as required to protect the safety of participants and staff. No refunds will be issued under these circumstances. We reserve the right to change the details of the event without prior notice. You understand that your entry fee is nonrefundable, and registration and bib numbers are non-transferable.
By submitting this entry, you acknowledge (or a parent or adult guardian for all children under 18 years) having read and agreed to the above release and waiver including the no refund policy.
