Comprehensive Care for Aging Adults

JCS’ Older Adult Case Management Program supports the safety, health, and independence of seniors throughout Miami-Dade County.

The program’s mission is to help older adults age in place by providing case management, in-home assessments, and coordinated referrals to community resources. Case managers develop personalized care plans addressing health, social, and financial needs—ensuring that each client can continue living safely and with dignity in their own home.

Each year, this program assists hundreds of older adults with accessing vital services, from transportation and homecare to emergency financial aid.

How to Get Help

Older adults or caregivers can call JCS Access at 305.576.6550 from 8:00 am–5:00 pm, Monday through Friday, to request an intake and speak with a case manager.
